First off we must pray for the family of the slain officer. I hate to hear of this news.

Please read the news headlines on the correction officer who was shot and killed by the wife of a prisoner he was escorting into the van at the courthouse in a small town. She had a handgun, walked up to him and shot him. He did not have a vest on.

Not to sound crass, but...
Am I being overly critical in thinking he should have had a vest on? He also had not had time to draw his own weapon - should he have been more aware/alert?

Speculating of course, but he may have thought being in a small town made him safe? Gives me one more reason to be vigilant.

There is a time to think and a time to act. This, gentlemen, is no time to think.

It seems to me that even bad guys have families and friends. Was the officer working alone? If so, I find it unconscionable that he'd be in a public place, responsible for a prisoner, with no one to watch his back.

Keep the lever gun handy (next to the revolver)
