Smith and Wesson recently issued a safety alert for M&P15-22 rifles. According to the alert, on some of these rifles, the breech face counter bore depth is not within manufacturing specification. Smith & Wesson advises that with the affected firearms, the lack of depth may allow the bolt, upon closing, to crush the rim of the case, causing the round to fire, cycling the bolt, and potentially resulting in multiple discharges without pulling the trigger.

If you have an M&P15-22, you can conduct a S/N search on the S&W website to see if your rifle may be defective.
